NDIS Support Coordination

Support coordination is a capacity-building support designed to help you make the best use of your NDIS plan. It enables you to connect with service providers, community supports, and government services while building your confidence and skills to manage and coordinate your supports independently.
Our support coordinators are dedicated to assisting NDIS members by understanding their needs and successfully executing their plans. Through close collaboration, we help you identify your goals and maximize your NDIS funding to live freely and comfortably. We assist you in finding high-quality services that ensure the highest standard of care.
Levels of Support Coordination
There are three levels of support coordination available under the NDIS:

This level helps you build the ability to connect with informal, community, and funded supports, allowing you to make the most out of your NDIS plan and achieve your goals. | A support coordinator will collaborate with you to ensure a balanced mix of supports, enhancing your capacity to: 1. Maintain relationships 2. Manage service delivery tasks 3. Live more independently 4. Participate in your community | This higher level of support is for individuals with complex needs requiring specialist intervention. A Specialist Support Coordinator will assist in managing challenges within your support environment and ensuring consistent service delivery tailored to your specific needs |
